When you click on the link to the left, you will see a page open up with the mail in order form. Print this form, fill it in with YOUR information at the top where indicated. Then fill in your order information. Be sure to click on the shipping link in the left hand column after you subtotal your order. Add in the appropriate shipping cost based on your subtotal of items. IF you live in the state of Washington, please make sure and add the appropriate amount of tax as indicated on the form. After you have a total, you're ready to send it in with your appropriate payment source. If you need assistance, please don't hesitate to email me :)
